Full-Time Employees

Full-time employees** (working 30 or more hours per week) may be eligible to enroll in medical*, dental, and vision benefits offered by Phoenix on the first of the month following 60 days of active service. You will also receive 100% Employer Paid Group Life and Accidental Death and Dismemberment Insurance of $50,000.

*If enrolled in one of our medical plans, you will be eligible to utilize our off-site clinic, One Medical, at no cost share to the employee.

Part-Time Employees

Part-time employees (working at least 20-29 hours per week) will receive 100% Employer Paid Group Life and Accidental Death and Dismemberment Insurance of $10,000 on the first of the month following 60 days of active service.

Medical

Phoenix offers eligible employees a choice between two medical insurance plans for coverage: a Traditional Preferred Provider Organization (PPO) and a Minimum Value Plan (meets the requirements for ACA-Affordable Care Act), administered by BlueCross BlueShield of Alabama.

One Medical Clinic

One Medical is an off-site employee clinic available to all employees enrolled in one of our medical plans at no cost share to the employee. They are a primary care practice that makes getting care anytime faster, easier, and more enjoyable.

Dental

Phoenix offers a Preferred Provider Organization (PPO) plan administered by Delta Dental. The plan is affordable and covers services* like preventive care (including regular checkups), fillings, bridges, crowns, and other dental services.

Vision

Phoenix offers vision insurance administered by VSP Vision Care. The plan offers employees coverage for routine eye exams, lenses and frames, and contacts. The vision plan coverage is nationwide and provides both in-network and out-of-network coverage. Your coverage goes further in-network. With so many in-network choices, VSP makes it easy to get the most out of your benefits. You’ll have access to preferred private practice, retail, and online in-network choices.

Group Life and Accidental Death and Dismemberment Insurance (AD&D)

Employer-paid Life and AD&D Insurance* is at no cost to the employee and administered by New York Life. Phoenix provides life insurance coverage of $50,000. For a covered accidental loss of life, your AD&D coverage amount is $50,000. For other covered losses, a percentage of this benefit will be payable. If still employed at age 75, the benefit amount will be reduced to 50%.

Retirement

Phoenix offers a voluntary 401(k) retirement plan administered by Ascensus. The retirement allows eligible employees* to make pre-tax elective deferral through payroll deductions. Eligible employees* may receive an employer match. Employee contributions are always 100% vested.

Contributions Limits

The IRS sets employee contributions limits for the 401(k) plan. The limitations apply to contributions made in a calendar year (January – December). The limit for employees under 50 participating in the retirement plan is $22,500. Employees aged 50 or older can contribute an additional $7,500, known as the catch-up contribution, equaling $30,000 for the calendar year. 

*Eligibility is based on employee type and the collective bargaining agreement (CBA) for those in a union, for participating and the employer match.
